MEDICARE pays 100% of home health care, therefore there is NO deductible or co-pay by the Patient. Patients may meet Medicare eligibility requirements if all the following conditions are met.

• Patient is under the care of a Physician
• Requires the skills of a Registered Nurse, PT or SPT
• Patient is Homebound

 

INSURANCE GUIDELINES may vary for home health benefits and we can assist you with this determination.

Follow along with the admissions process

Home Health Admissions:

• Complete evaluation / assessment of patient's needs.

• Coordination of services with ongoing communication with your Physician.

• Nurse on Call 24/7

 

REFERRALS can be made by the patient, your Physician, family or friend, or Hospital Discharge Nurse. Once we receive the referral we will contact your Physician to obtain an order for home health services. Close contact is maintained with your Physician to insure optimum continuing care.
